State Police Investigating Fatal Crash in Bolton

Today at approximately 2:45 a.m. Troopers assigned to State Police-Leominster responded to reports of a two vehicle crash, with serious injuries, on Route 495 southbound in the area of mile marker 68.2 in Bolton.

Upon their arrival they observed a 2012 Honda Civic, operated by John Keeley, 41, of Clinton, that had struck the rear of a 2016 International tractor-trailer. Keeley was the sole occupant of the Honda and suffered severe injuries during the collision. The tractor-trailer was occupied by a 56-year-old man from Woonsocket, R.I., who was treated on-scene for minor injuries. The truck was stopped in the breakdown lane at the time of the crash.

Both Bolton and Berlin Fire Departments and EMS responded to the crash. Members of the Berlin Fire Department determined Keeley deceased at the scene. Examiners assigned to the Office of the Chief Medical Examiner responded to the scene as well.

The exact cause and circumstances of the crash remain under investigation by Troopers assigned to State Police-Leominster, State Police Commercial Motor Vehicle Enforcement Section, Collision Analysis Reconstruction Section, and Crime Scene Services. Troopers were assisted on scene by MassDOT, Office of the Chief Medical Examiner, Bolton Police Department, and Bolton and Berlin Fire & EMS.
